# Beauty Service Intake Questions: Examples and Boundaries

FindSlot Team · Published Oct 18, 2026 · Updated Oct 7, 2026

Beauty service intake questions should help your team prepare for an appointment, explain a service choice or identify a question to discuss before the visit. Ask for information that changes what you do. Leave out questions that create more reading without improving the appointment.

For a salon or beauty studio, useful preparation might mean understanding the desired finish, knowing whether the client is new to the business or checking that they chose the right service. These are operational questions. A general booking form should not become a place for collecting clinical histories, diagnoses or treatment records.

## Start with the decision behind each question

Before adding a question, complete this sentence: "When a client answers this, the team will..." If you cannot name the action, reconsider the question.

"What result would you like?" can help the provider prepare a conversation. "Which of our services have you used before?" can reveal whether the client needs a simpler explanation. Asking for someone's occupation usually does neither unless a specific service genuinely depends on it.

Use this planning table before writing the form:

**Desired finish**

- **Useful question:** What would you like to achieve at this visit?
- **Action the answer supports:** Start the service discussion with the client's goal

**Service uncertainty**

- **Useful question:** Are you unsure which service to choose?
- **Action the answer supports:** Arrange a conversation before confirming the scope

**First appointment**

- **Useful question:** Is this your first visit to our studio?
- **Action the answer supports:** Explain arrival and consultation expectations

**Reference preference**

- **Useful question:** Would you like to discuss a reference image at the visit?
- **Action the answer supports:** Prepare time for a visual conversation

**Practical support**

- **Useful question:** Is there an arrival or communication preference you would like us to know?
- **Action the answer supports:** Agree practical arrangements directly

These are suggested question examples, rather than a list of specific field types in FindSlot.

## A short question bank for different beauty services

Keep the questions connected to the service the person actually selected. A haircut client should not work through a manicure questionnaire simply because the business offers both.

### Haircut or styling appointment

- What would you like to change or maintain at this appointment?
- Is there a preferred length or finish you would like to discuss?
- Are you booking a regular maintenance visit or considering a different style?

The answers provide a starting point. "Keep the length, change the shape" helps staff prepare more than a vague requirement to describe everything about the client's hair.

### Nail service appointment

- Are you looking for a simple finish or a design to discuss with the provider?
- Will you need help choosing the appropriate service from our menu?
- Is there a reference style you plan to bring?

If an answer suggests that the selected appointment may need a different service or more time, discuss it before agreeing to a change. An intake answer should not silently add a service the client did not request.

### Makeup or occasion styling

- What is the occasion and preferred overall look?
- What time do you need to leave the studio?
- Are there reference images you would like to discuss?

Use the departure time as conversation context. Check the actual appointment length and availability before promising that the visit will fit a deadline.

## Keep service selection separate from consultation

An intake question can reveal uncertainty, but it cannot replace a proper conversation about scope. A client who wants a major change may need a consultation before the main service is agreed.

For example, imagine a client books a short styling appointment and writes, "I want a completely different look for an event." A useful response is to clarify the goal, explain what the selected appointment includes and agree the appropriate next step. The form has helped uncover a mismatch; it has not resolved it.

Use the [beauty studio consultation booking guide](https://findslot.ai/beauty-studio-consultation-booking) when deciding whether a separate consultation is the clearer path. For a color-specific visit, the [salon color consultation checklist](https://findslot.ai/salon-color-consultation-checklist) covers a narrower preparation workflow.

## Write questions that clients can answer quickly

A strong question uses ordinary language and makes its purpose obvious. "What would you like us to focus on?" is easier to answer than "Provide all relevant service objectives."

Avoid asking the same thing twice. If the booking already identifies the service, provider and time, do not ask the client to type those details again without a specific reason. Repeated questions can also produce conflicting answers.

Explain optional questions honestly. An optional preference should not read as a condition for being served. If staff need to talk before accepting the requested scope, say so in the service description rather than hiding that requirement in a long form.

A short introduction could read:

> These questions help us prepare for your visit. Share your preferred result and anything you would like to discuss about the service. Please keep answers relevant to appointment preparation.

This is sample copy to adapt to your business, not an automatic FindSlot message.

## Review answers before they become assumptions

Assign a person to read the preparation information before the appointment. Otherwise, a carefully written questionnaire creates expectations your team may not meet.

The reviewer should check whether the answer affects service choice, time or communication. If it does, clarify the arrangement with the client. Record only the useful outcome of that conversation, such as "Client will bring a reference photo; discuss shape before starting."

Use [client profiles and appointment history](https://findslot.ai/client-management-service-businesses) for existing booking context. A repeat client's previous visits can inform the discussion, but a past preference should not be treated as their current instruction.

## Test the question set with a realistic booking

Read the questions on a phone, with the selected service in mind. Ask a colleague to answer as a first-time client and explain any words they find unclear.

Then use this editing checklist:

1. Each question has an identifiable preparation action.
2. The wording fits the selected service.
3. Existing booking details are not requested again unnecessarily.
4. Optional context is clearly presented as optional.
5. Staff know who reads the information and when.
6. A question about scope leads to a conversation, rather than an unannounced booking change.
7. Sensitive or unrelated information is excluded.

If a question generates vague answers, improve the wording before adding more questions. If nobody uses the answers, remove or replace it.

## How this fits FindSlot

FindSlot's [current pricing page](https://findslot.ai/pricing) lists basic intake forms on Pro and Enterprise. Use the available controls in your account to support a short preparation workflow; the examples above are a content plan, rather than instructions for a particular field editor.

The [booking page](https://findslot.ai/features/booking-page) still needs clear service descriptions, because intake questions cannot repair an unclear menu. Start with a service the client understands, ask only what helps staff prepare and leave detailed discussion for the right conversation.
